Filling Out a Form in French: Name, Address, Date of Birth and More

8 min readUpdated September 24, 2026By

Getting Started

Newcomers to Canada fill out dozens of forms in their first months: a health card, a bank account, school registration, a lease, a job. In Québec these forms are often only in French, and everywhere else federal forms always have a French version. The TEF/TCF reading section also uses forms and official notices.

This lesson covers the words found on almost every form, and ends with a filled-in example.

1 Your identity

French English Example Example (EN)
le formulaire form Remplissez ce formulaire. Fill out this form.
le nom (de famille) surname, last name Mon nom de famille est Singh. My last name is Singh.
le prénom first name Mon prénom est Harjit. My first name is Harjit.
la date de naissance date of birth Quelle est votre date de naissance ? What is your date of birth?
le lieu de naissance place of birth Lieu de naissance : Amritsar, Inde. Place of birth: Amritsar, India.
le sexe sex Cochez le sexe : M ou F. Tick your sex: M or F.
la citoyenneté citizenship Citoyenneté : indienne. Citizenship: Indian.
la pièce d’identité ID document Vous avez une pièce d’identité ? Do you have ID?
le numéro d’assurance sociale (NAS) Social Insurance Number (SIN) L’employeur demande votre NAS. The employer asks for your SIN.
la carte d’assurance maladie health card J’ai reçu ma carte d’assurance maladie. I got my health card.

An important point about names: on a form, Nom usually means your surname, not your full name; a full name is nom complet. If your passport shows Kaur or Singh as your surname, write exactly that, so your name is identical on every document.

2 Address and contact

French English Example Example (EN)
l’adresse address Écrivez votre adresse complète. Write your full address.
le numéro d’appartement apartment number Numéro d’appartement : 304. Apartment number: 304.
la ville city Ville : Laval. City: Laval.
la province province Province : Québec. Province: Québec.
le code postal postal code Mon code postal est H7N 1A1. My postal code is H7N 1A1.
le numéro de téléphone phone number Laissez votre numéro de téléphone. Leave your phone number.
le courriel email Mon courriel est sur le formulaire. My email is on the form.
la personne à joindre en cas d’urgence emergency contact La personne à joindre est mon frère. My emergency contact is my brother.
le lien de parenté relationship (family) Lien de parenté : épouse. Relationship: wife.

Note: in Québec, email is almost always courriel; in France e-mail or mail is also common. A French address puts a comma after the number: 45, rue Principale, app. 304.

3 Marital status (état civil)

French English Example Example (EN)
l’état civil marital status État civil : marié. Marital status: married.
célibataire single Je suis célibataire. I’m single.
marié / mariée married Elle est mariée depuis cinq ans. She has been married for five years.
conjoint de fait / conjointe de fait common-law partner Mon conjoint de fait vit avec moi. My common-law partner lives with me.
divorcé / divorcée divorced Il est divorcé. He is divorced.
veuf / veuve widowed Ma grand-mère est veuve. My grandmother is a widow.
les personnes à charge dependants Combien de personnes à charge avez-vous ? How many dependants do you have?

5 Date formats

Every form states its format. The letters come from French words: A = année (year), M = mois (month), J = jour (day).

  • AAAA-MM-JJ → 1990-03-05 (many Canadian government forms)
  • JJ/MM/AAAA → 05/03/1990

The wrong order turns 5 March into 3 May, so check the letters every time.

6 A filled-in example

FORMULAIRE D’INSCRIPTION – COURS DE FRANÇAIS

Nom : KAUR · Prénom : GURPREET Date de naissance (AAAA-MM-JJ) : 1991-11-08 · Lieu de naissance : Jalandhar, Inde Sexe : ☐ M ☒ F Adresse : 2150, boulevard Saint-Laurent, app. 12 · Ville : Montréal · Province : Québec · Code postal : H2X 2T4 Téléphone : 514 555-0147 · Courriel : [email protected] État civil : ☐ célibataire ☒ mariée ☐ conjointe de fait Langue maternelle : punjabi · Autres langues : hindi, anglais Personne à joindre en cas d’urgence : Amandeep Singh · Lien de parenté : époux Signature : G. 7 At the counter

Sometimes a clerk fills in the form and asks you:

  • Quel est votre nom de famille ? — Kaur.
  • Pouvez-vous l’épeler, s’il vous plaît ? — K, A, U, R. (For letter names, see Alphabet and Sounds.)
  • Quelle est votre date de naissance ? — Le 8 novembre 1991.
  • Vous avez une pièce d’identité ? — Oui, voici mon passeport.
  • Signez ici, s’il vous plaît.

Practice

What would you write in these boxes? Answer with your own details.

  1. Prénom : ______
  2. Date de naissance (JJ/MM/AAAA) : ______
  3. État civil : ______
  4. Code postal : ______
  5. Ville : ______

Then: can you leave a facultatif box empty? And an obligatoire one?

Answers

Facultatif = optional, so you can leave it empty. Obligatoire = required, so you must fill it in. JJ/MM/AAAA means day first, then month, then the four-digit year.

FAQ

Does 'Nom' on a form mean my full name?

No. On a form, Nom usually means your surname, and Prénom your first name. For Harjit Singh: Nom = Singh, Prénom = Harjit. Write exactly what is on your passport.

What does 'conjoint de fait' mean?

It is Canada's common-law partner: a couple who have lived together in a conjugal relationship for at least one year without marrying (the federal immigration definition). It appears on most government forms.

How do I write the date on a form?

Follow the format printed on the form. AAAA-MM-JJ means year-month-day (2026-09-24); JJ/MM/AAAA means day/month/year (24/09/2026). A = année, M = mois, J = jour.
